When Open Surgery Is Used
Open hernia repair remains a standard and effective approach. It is often preferred for large or complex hernias, when the abdominal cavity cannot be safely accessed with minimally invasive techniques, when a previous laparoscopic or robotic surgery has failed, or when conversion from keyhole surgery is needed. Open repair can also be performed under local or regional anaesthesia when general anaesthesia is not suitable.
Open Inguinal Hernia Techniques
Common techniques include herniotomy, Bassini, Shouldice, Desarda, Lichtenstein (mesh), and trans-inguinal preperitoneal repair. Your surgeon will select the best option based on your hernia type, age and overall health. Mesh repair (e.g. Lichtenstein) is widely used and has low recurrence rates.
Open Ventral Hernia Techniques
For ventral and incisional hernias, techniques include onlay, inlay, sublay and underlay repair; Rives-Stoppa; posterior component separation (PCS/TAR); and anterior component separation (ACS). Large hernias may require component separation to restore the abdominal wall. Dr. Sharad Sharma has extensive experience in these techniques.
